Article VI. VEHICLES FOR HIRE |
§ 13-138. Application; form; fee.
(a)
An application for a vehicle for hire permit shall be filed with the county administrator annually for each vehicle being operated upon forms provided by the county and such application shall contain the following information:
(1)
Name and address of applicant; if a corporation or partnership, the names and addresses of the corporation officers and directors, or partners if a partnership (both general and limited, as applicable) shall be furnished. If a joint venture, syndicate or other business entity, the names and addresses of all participants.
(2)
The number of vehicles to be operated or controlled by the applicant and the locations of any proposed depots, terminals, stations, or offices.
(3)
The color scheme or insignia to be used to designate the vehicle or vehicles of the applicant.
(4)
Such further information as the county administrator may reasonably require.
(5)
A fee as determined by the board of county commissioners shall be paid to the county at the time of filing the application for a vehicle for hire permit.
(6)
The applicant shall arrange a mutually convenient time with the county administrator to present each vehicle at a location to be designated by the county administrator for inspection to determine compliance with the standards of section 13-145.
(b)
The application for driver's certificate shall be filed with the county manager on forms provided by the county and such application shall be verified under oath and shall contain the following information:
(1)
The applicant is at least twenty-one (21) years of age.
(2)
The applicant's current State of Florida chauffeur's license number and original date of issue.
(3)
The experience of the applicant in the transportation of passengers for hire.
(4)
A driver's certificate application fee of fifty dollars ($50.00) shall be charged at the time of making such application. Upon approval of the application, the county manager shall issue a driver's certificate to the applicant which shall bear the name, address, age, signature, and photograph of the applicant. Such certificate shall be in effect for a period of three (3) calendar years, unless earlier revoked.
