§ 9-156. Application for permit.

Any person seeking a permit pursuant to section 9-155 above must first submit an application containing the following elements, at a minimum, to the HRS local health unit of the county.

(1)

Copy of one (1) or more of the following, as may be applicable pursuant to Chapter 17-7 or Chapter 71-640, FAC; (a) DER "Land Application Field Package for Grade I Domestic Wastewater Residual"; or (b) Copy of "General Permit Application for Grade II Domestic Wastewater Residual"; or (c) Copy of DER "Agricultural Use Plan"; or (d) Copy of DER "Dedicated Site Plan."

(2)

A letter from the landowner authorizing application on his/her/its property and granting access to county employees for inspections.

(3)

A copy of letter(s) mailed pursuant to subsection 9-153(2) above with the certified mail, return receipt attached indicating the date of mailing.

(4)

A site plan of the proposed landspread/application site.
