§ 12-50. Form.

Latest version.

Each permit required by the provisions of this division shall be on a form developed by the DPS and shall be signed by the chairman of the board of county commissioners. Each permit shall contain at a minimum, the name and address of the applicant, a description of the vehicle or vehicles to which it applies and such additional terms, conditions, provisions and limitations as were authorized in the approval process.
